Pulmonary thromboembolism and pericardial effusion as cardiovascular complications of COVID-19 infection

Systemic activation of coagulation and pulmonary thrombo-inflammation with local vascular damage caused by SARS-CoV-2 infection increases the risk developing thromboembolic complications: stroke, arterial thrombosis (pulmonary thromboembolism) deep vein thrombosis. Myopericarditis may occurs in COVID-19 patients as part or after onset respiratory symptoms. Minor pericardial effusions up to 1 cm that accompany involvement are common. In our patient during hospitalization due bilateral pneumonia SARS-Cov-2 virus a routine control D-dimer, elevated values 2.3 fold higher than reference range were observed, biomarkers inflammation. She had symptoms no pronounced clinical would indicate thromboembolism. MSCT angiography was performed low-risk thromboembolism confirmed. Anticoagulant therapy started - therapeutic doses low molecular weight heparin (enoxaparin), which extended discharge from hospital DOAC (Rivaroxaban) according protocol for treatment At examination 3 weeks, pericarditis moderate effusion determined. (DOAC) inclusion colchicine effusion. After months mild symptoms, bronchopneumonia right lung developed slightly inflammation normal D-dimer. With prescribed antibiotic current (DOAC colchicine), there withdrawal regression reduction ventricular overload. follow-up 5 hospitalization, complete confirmed D-dimer values. It is advisable exclude anticoagulant continued low-dose aspirin therapy.
